DISD Health Service Notification
Denison ISD is committed to supporting the health and well-being of every student. Our nurses provide a wide range of health services that support academic success by managing both acute and chronic conditions. In accordance with Texas law (TEC §38.036), we are providing you with a list of all health-related and health-care services that may be provided to students, at Denison ISD campuses, by licensed healthcare professionals or other authorized personnel. Health-related services will utilize an opt-out parent procedure, while health-care services will require express, written consent.
Please note that essential caretaking actions and emergency care will be provided regardless of consent. “General care-taking”, as defined by TEA, is noninvasive actions to address minor student claims of feeling unwell or adult observations of a child being physically unwell. This type of care, along with emergency care, will always be provided to ensure student safety and well-being of all students during the day. General care-taking can include:
- Basic assessment (such as taking a temperature or looking at a sore throat)
- Minor student care actions (applying a band-aide, wiping a bloody nose, providing feminine hygiene products, offering a change of clothing when available, etc)

Health-Related Services (Opt-Out Services)
Denison ISD Health Related Services Opt-Out Form
As defined by TEA, “Health-Related Services” are typically short-term, noninvasive activities designed to promote a student’s mental, emotional, or physical health, but are not considered a health-care service. Students will automatically have access to health-related services unless a parent or guardian submits an opt-out request. If you wish to decline certain health-related services, please complete the Opt-Out Form and return it to the campus nurse. Opt-Out forms are effective until the end of the school year in which obtained. You may update your preferences at any time by contacting the campus nurse or school office to request a new Opt-Out form or revoke your previous opt-out selections in writing. Once received, your child will be eligible to receive the updated service selection moving forward.
Health-Related Services include:
- Health screenings (i.e. vision, hearing, spinal, etc)
- First aide care
- Assessment and comfort care for short-term illness, injury, or physical/emotional symptoms while at school (i.e. ice/heat pack, rest, etc)
- Wellness promotion and education (i.e. handwashing, covering sneeze, symptom management)
Health-Care Services (Opt-In Services)
“Health-Care Services” are formal medical interventions that are planned and designed to treat a specific condition. These services, except in the case of emergencies, require express, written consent before they can be provided. These are handled on a case-by-case basis. Please reach out to your campus nurse, for service specific consent forms. Once you have consented to a health-care service, you may update or revoke your consent at any time by contacting the campus nurse.
Health-Care Services include:
- Administration of medication and supervision of self-carry medication (requires physician order)
- Medical treatments (requires physician order)
- Medical procedures (requires physician order)
- Ongoing assessment and intervention for chronic illness and/or post hospitalization care (requires physician order)
- Telehealth Services
- Administration of a well-being questionnaire or health screening form (copy to be provided to parent)
